The Admission Forms can be obtained during banking hours from Habib Bank Ltd, or Askari Bank Ltd., QAU Branches by paying a fee for Rs.700/- OR Click here to download and can be submitted alongwith a bank draft amounting to Rs.700/- in favour of Treasurer, Quaid-i-Azam University.
